A caregiver is someone who is optionally designated by a qualifying patient to cultivate or to assist the qualifying patient’s medical use of marijuana. A caregiver must have a designation form filled out by the qualifying registered patient and have agreed to assist the qualifying patient with the medicinal use of marijuana. A caregiver is required to register with the Department of Health and Human Service (DHHS) except if the designated caregiver is within the household. A caregiver may only assist no more than five qualifying patients at a time.
Requirements
- Must be at least 21 years of age or above.
- Have no background of any qualifying drug offense.
- Must be registered with the Department of Health and Human Service (DHHS) by submitting a completed form and required registration fee.
Second Caregivers may be applied to the following:
- A minor registered patient.
- An incapacitated registered patient.
- A registered patient who is in a nursing or hospice care facility.
Becoming a Caregiver
Step 1: The patient must be a cardholder or registered for the medicinal use of marijuana.
Step 2: The patient must designate a caregiver as his personal caregiver in assisting medicinal use of marijuana.
Step 3: The patient will give the designated caregiver a form to register with the department (DHHS).
