Basic Legal Requirements
- Must have a valid Maine identification
- Written consent of parent or legal guardian

Proof of Qualifying Conditions
- Patient must be suffering from any qualifying symptoms conditions
- Must be certified by a qualifying doctor
Complete the Paperwork
- All required forms must be completed and approved by Maine’s Department of Health and Human Service (DHHS)
- Designated caregiver and dispensary forms must be signed
